If you would like to register your child to attend either of the clubs on a permanent weekly basis or if you would like to alter your child's regular sessions, please complete the relevant form below and hand to the school office.
If your child is new to the After School Club, you also need to complete a password form for security purposes. Please see below.
If you would like to make any changes to your regular booked sessions, eg. changing days or cancelling sessions, you must complete the form below, giving 30 days notice. If we do not receive a written request to alter or cancel sessions, your child will remain on the register and unattended sessions will still be charged.
